La documentazione da allegare al modulo deve essere accompagnata da una traduzione in lingua italiana o inglese munita della dichiarazione sostitutiva atto di notorietà oppure da una traduzione autenticata redatta da un traduttore terzo riconosciuto dallo Stato membro di origine o da altro Stato dell’Unione.
